Departments shifted from reactive crisis-only communication to ongoing healthcare facility partnerships
Health departments developed innovative solutions, including equity scoring systems, comprehensive data frameworks, and rural-appropriate toolkits
Established sustainable foundations enabling departments to continue equity-focused work beyond the grant period
