Action Plan to Reduce Chronic Disease in Rural Montana
Summary
Chronic diseases are a leading cause of illness and death in Lewis and Clark County, MT. We partnered with woman-owned consulting group, Yarrow, LLC, the County Health Department, residents, and partners to develop a Social Determinants of Health Accelerator Plan. The work included:
Designed and facilitated strategic planning meetings with task force
Facilitated the development of an action plan
Supported writing and editing the final plan
Provided recommendations for implementation and sustainability
Presented the plan's goals and objectives to the County Health Coalition
A clear path forward enabled the county to create culturally responsive policies, systems, and environments that prioritize health equity as a crucial factor in chronic disease prevention and management.
